Return-to-work planning

A written plan for your first days back, built from the tasks of your job and what you can do now.

This care is for you if…

  • You are close to going back and want a plan

  • Your employer offered light or modified duty and you want to know what fits

  • Your doctor set limits and you want to understand them

  • You went back before and it was too much

What a visit looks like

  • What we do

We compare your job's tasks with your test results, then write a plan: which tasks to start with, which to change for now and how to build up. With your OK, we share it with your employer and your adjuster.

  • What decides how many visits

What your doctor allows, what the job's tasks ask, what your test shows and what your employer can modify. The plan changes as you do.

  • Who treats you

A physical therapist writes the plan. Our case coordinator sends the updates to the people your case names.

    Meet the team
  • How it is paid for

Usually with the rest of your care, by the payer on your case. We tell you before you start.

    How fees work
  • Chest pain, trouble breathing, a sudden severe headache or pain, new weakness or numbness, loss of bladder or bowel control, or a fall with a head injury needs emergency care now. Call 911.

Questions about this care

Do you talk to my employer?

With your OK, and only about what the case needs: what you can do and what is still limited. Not what you tell us in private.

What if my job can't change?

We write down what the job asks and what you can do now. Your doctor and your employer decide what comes next. We plan around what is possible.

Do you decide when I go back?

No. Your doctor decides about work limits, and your employer decides about the job. We give you both a clear picture of what you can do.

Request a visit

We hear about your injury at your first visit, not in the form.
